Meridian at Home: Keeping You Living Well and Independently
You shouldn’t have to choose between your health and your independence. Fortunately, you don’t have to with Meridian at Home. This program can deliver everything from state-ofthe-art medical services to assistance with activites of daily living in the comfort and privacy of your own home.

If you have a long-term medical problem that requires high-level care or need assistance with errands or meal preparation, the choices don’t always look good. For many, getting the necessary care means entering a nursing home or other live-in facility. This involves leaving your home and family, facing the high costs of full-time inpatient care, and, perhaps worst of all, losing your sense of independence and control over your life.

ALL-Inclusive Services
Meridian at Home offers the full range of home-care choices for patients and their families. Patients can receive acute nursing care, rehabilitation therapy, medical equipment, pharmacy services, and much more.

"I didn’t want to go to a nursing home," says Ed Brigham, a Meridian at Home patient suffering from complications of diabetes, who has used the program for more than two years. "I wanted to stay in my own environment, where I feel comfortable. With these services, I’m able to do that."

The program also offers state-of-the-art remote-care technologies, including medication reminders and alert systems that notify family members if patients haven’t eaten or taken their medication. Patients can even use remote monitoring equipment to transmit their vital signs and health information to clinicians, so that any developing problems can be identified and dealt with immediately.

Meridian at Home also provides assistance beyond clinical care. "If a patient needs a home health aide, we’ll provide that," says McCudden. "If they need transportation or in-home care around the clock, 24 hours a day, seven days a week, we can do that too. Whatever they need to live safely and comfortably at home, we can deliver it."
